CASE MANAGER, Senior Services
THIS IS NOT A HANDS ON CARE ROLE. IT IS A MANAGEMENT ROLE.
Provide status assessment and arrange for appropriate support by overseeing THE CARE PLAN for seniors in our community.
Jewish Family & Children’s Services of Northern NJ (JFCS), is a non-profit organization serving Bergen, Passaic and Hudson Counties in New Jersey.
Through an initial formal assessment, periodic home visits, and GENERAL OVERSITE of each case, the Case Manager arranges for appropriate support and oversees the care plan, thus enabling the senior to remain in their homes safely.
THIS IS NOT A HANDS-ON CARE POSITION! … rather it is a management position to OVERSEE AND MANAGE the overall CARE PLAN for seniors, including Holocaust Survivors.

At initial face-to-face visit, facilitate assessment to determine eligibility
Ensure that all relevant forms (and registration) are signed and submitted
Develop a Plan of Care with client and/or caregiver
Assist client to obtain/access both formal and informal services
Ensure monthly contact with clients
Keep service logs and accurate progress notes in our data base
There is mileage reimbursement for any travel
You are an empathetic and responsible professional; experience with seniors a +!
Must have a Bachelor’s Degree in human services related field from an accredited college or university
Two years of related experience, including internships
Comfortable with databases and other computer programs (Word, Excel)
Good assessment skills, ability to multi-task
Speaking Russian would be an amazing plus, but totally not essential.
